Insurances We Accept

​

Plateau Behavioral Health Associates accepts the following insurances:

Kaiser PPO, First Choice Network of Washington, Aetna, Regence BlueShield of Washington, United Healthcare, Cigna, Premera Blue Cross of Washington, Oscar Health, Oxford, Aetna & Optum.  We will be adding other insurance providers soon.

We do not currently accept Medicare or Medicaid, however will be accepting them in the near future.  If you would like us to contact you when we are seeing Medicare or Medicaid patients, please call us. 

​

Office hours are by appointment, Monday through Friday 8 AM to 6 PM

​

Services we provide include Psychiatric Evaluations & Assessments, Therapy and Medication Management. 

​

Before Your First Visit

​

We will email you intake forms to be fully filled out and returned before your appointment. These forms must be returned to us 48 hours prior to your appointment.  

 

If you are unable to complete the forms electronically please contact us and we can make other arrangements.

How do I pay?

​

Acceptable forms of payment for self-pay, deductible owed and co-pays are exact-amount cash, or a credit or debit card on file. Payment is due at the time of visit and will be billed to your card on file.  

​

We do accept self-pay 

  • $300 for initial evaluation (self-pay)

  • $150 for follow-up visits (self-pay)

​

If you are paying out of pocket and have insurance that is out of network, we can provide you with an itemized receipt/super bill that may allow you to get reimbursement from your insurance company, usually between 60-80% after your deductible is met
